Executive Summary

The Alabama Department of Mental Health (ADMH) proposes to have 20 AmeriCorps members who will engage in outreach, counseling, education, and referral in Cullman; Marshall; Marengo; Mobile; Jefferson; Montgomery; St. Clair; Etowah; and Tuscaloosa counties. At the end of the first program year, the AmeriCorps members will be responsible for ensuring a statistically significant and successful health care/social service referral rate; and, a demonstrated increase in improved health-related behaviors among the target population, as evidenced by a reduction in 30-day usage of opioids. In addition, the AmeriCorps members will leverage an additional 0 volunteers. This program will focus on Healthy Futures. The CNCS investment of $276,594 will be matched with $133,258, $133,258 in public funding and $0 in private funding.
