Executive Summary

The Area Agency on Aging, Region One proposes to expand the In-Home Support Service component of the current Los Ancianos AmeriCorps Encore program to become Caring Circles by having 25 MSY AmeriCorps Encore members (15 new, 10 existing) serve in homes and neighborhoods of isolated seniors on the waiting list for home and community based services in Maricopa County, AZ by providing advocacy through a new Caring Circle program that will include initiatory home visits; access to Agency staff who can provide access to resources; telephone reassurance; and build neighborhood awareness and volunteer support for older adults in need of home and community based services who are currently on one or more assessment and/or program waiting lists and are not currently receiving services. At the end of the first program year, the AmeriCorps Encore members will serve an estimated 1,500 older adults through indirect services, the promotion of awareness of and care for isolated elders, and telephone support that will help older adults to remain safe and healthy in their own homes. In addition, the AmeriCorps members will leverage an additional 3,000 volunteers recruited from neighborhood churches, schools, businesses, and residents who will be engaged in providing service to home bound older adults in need of help with personal care, access to adequate food, transportation, activities, and companionship. This program will focus on the CNCS focus area of Healthy Futures through services that promote Aging in Place and focus on the funding priority for Encore programs. The CNCS investment of $640,703 will be matched with $241,921 from unrestricted Agency funds from state funding sources.
