Executive Summary

The Utah Healthcare Corps, an AmeriCorps program of the Association for Utah Community Health, proposes to have 50 full-time and 10 minimum-time AmeriCorps members improve health outcomes and access for low-income and medically vulnerable individuals in Utah through comprehensive case management services, including health education for individuals at risk for opioid and prescription drug abuse. At the end of the first program year, at least: 8,640 individuals will access primary healthcare services; 5,000 individuals will receive health education specific to reducing and preventing prescription drug and opioid abuse; 5,800 individuals will enroll in healthcare affordability programs; and 25,000 individuals will receive information on health insurance and access to care. In addition, the AmeriCorps members will leverage an additional 200 volunteers who will engage in case management and interpretation services.

This program will focus on the CNCS focus area of Healthy Futures to improve access to primary and preventive healthcare for communities including reducing and preventing prescription drug and opioid abuse. The CNCS investment of $665,195 will be matched with $681,286 - $150,000 in public funding and $531,286 in private funding.
