LDA Minnesota will utilize a planning grant to develop an AmeriCorps program which will engage AmeriCorps members to support the transition to life beyond K-12 education for youth and young adults with disabilities in the Minneapolis and St. Paul metropolitan area. AmeriCorps members will educate and mentor students who face life challenges due to their disability through a variety of evidence-based transition planning practices, community-based experiences and intensive instruction resulting in graduation from high school and successful transition into a workplace, post-secondary institution, or vocational education program. Our work will take place in area high schools serving special education students and throughout the community, to provide experiential learning. Program activities will primarily be in the CNCS focus area of Education, through two of Americorp's established Service categories: job preparedness/school to work and college access and success.
