NARBHA's overall objective is to improve the health and wellness of low-income individuals in rural northern Arizona and to strengthen and support non-profit agencies to remain competitive and successful. NARBHA is a community- based non-profit organization that maintains a contract with the State of Arizona to manage and oversee Medicaid-funded behavioral health services in the mostly rural northern Arizona counties. NARBHA has developed a Health Integration Program which supports the integration of behavioral health services with physical health care services.
NARBHA will build the capacity of eight small and mid-sized non-profit organizations by providing a designated Health Integration Program Manager who will assist the eight organizations in building capacity by developing and implementing performance management systems. The Health Integration Program Manager will provide Best Practice tools, training, policy development, technical assistance, resource linkage, needs assessments and data collection methods tailored to the needs of each organization.
Approximately 2,100 individuals with multiple co-morbid physical and behavioral health conditions will be served by the Health Integration Program through eight providers at 29 sites in rural and underserved locations in northern Arizona. Positive outcomes will include increased capacity of the non-profit network to assist participants in the management of chronic conditions, reduce healthcare costs and improve life expectancy and well being of people served.
