Community Counseling & Mediation (CCM), a 25 year old minority-led not-for profit 501 (c) (3) agency in Brooklyn with over 30 contracts and 20 programs and an operating budget of $11 million is pleased to submit this proposal in response to the AmeriCorps State and National Grants FY2011 funding opportunity to provide a Leadership & Educational Attainment Program in Community Districts 8 (Crown Heights), Community District 9 (Bedford-Stuyvesant) and Community District 16 (Brownsville) in Central Brooklyn, New York. These targeted communities are unfortunately shrouded by depressed economic conditions, high rates of poverty, crime, substance abuse, teen pregnancy and parenting, high rates of child abuse and neglect, high drop-out rates and/or low scholastic achievement and widespread HIV/AIDS infestation and overcrowding. The site addresses of the service placements are as follows: New Life of NYC, located at1200 Dean Street, Brooklyn, New York 11216; 810 Classon Avenue, Brooklyn, NY 11238; P.S. 335 / 584 Rochester Avenue, Brooklyn, NY 11213; Human Rights located at 600 Kingston Avenue Brooklyn, NY 11203; The School for Democracy & Leadership located at 600 Kingston Avenue, Brooklyn, NY 11203, Clara Barton High School located at 901 Classon Avenue, Brooklyn, NY 11225, and Brooklyn United for Innovative Local Development (BUILD), located at 485 Hudson Avenue, Brooklyn, NY 11201.
